Hello,
I’m Amber the founder, owner, and provider at Amber Bahm Family Practice.
My journey in healthcare started in high school when my Grandma was diagnosed with ovarian cancer. I became a caregiver and knowledge seeker as we poured over her test results and medical notes to have a better understanding of her diagnosis and prognosis. From there my educational pursuits have taken me from Nebraska to Alaska and back. I graduated Magna Cum Laude from the University of Nebraska Medical Center and completed my Doctor of Nursing Practice in 2018 at Creighton University. I moved to Arizona in 2018 to pursue my dream of becoming part of and caring for a small community. I poured my heart and mind into the care of patients at the Community Health Center of Yavapai for nearly 8 years, earning multiple awards during my tenure there including the 2022 Arizona Star for my work on diabetes management from the National Organization of State Offices of Rural Health. I was also delighted to receive Nurse Practitioner of the year for the local newspaper Kudos in 2024 and 2025. I also hold an additional board certification in advanced diabetes management. I am currently enrolled as ASU obtaining an additional certification in psychiatry.
I believe in relationship based medicine. It is important that I know you as a human being to best care for you. I believe the opportunity to be healthy is something that should be available to everyone. I never take for granted the awesome responsibility it is helping others to make decisions about and to care for their own health. My goal for every patient is to help them live as well as they can for as long as they can. I am so thankful to be a part of the Cottonwood community and look forward to serving my prior patients from the Community Health Center as well as any community member looking for a provider to partner with them in navigating their health journey.
Hi, my name is Juriana Ruiz-Bustamante, and I am a newly graduated Medical Assistant with a passion for caring for others. I began my healthcare journey in 2021 as a caregiver, working closely with elderly patients and children with special needs. That experience taught me the importance of patience, compassion, and making people feel safe and comfortable in their own homes. In 2022, I continued working with special needs children in a public school, where I saw how important support and understanding are not only for the children, but for their families as well. Those experiences confirmed that healthcare was the path I wanted to follow. I previously worked with Amber at the Community Health Center of Yavapai.

Being there for them inspired me to pursue a career where I can support patients facing similar challenges and make sure they never feel alone in their care. As a mom of one and someone who is bilingual, I understand the importance of patience, communication, and making people feel understood. My goal is simple: to make sure every patient feels cared for, respected, and supported.

With more than 8 years of experience as a medical receptionist, I have built a reputation for my welcoming personality, compassion, and commitment to providing excellent patient care. I have had the pleasure of working alongside Amber for over 8 years, building lasting relationships with both colleagues and patients. In addition to my administrative experience, I have been a licensed massage therapist and aesthetician in Arizona for more than 15 years.
